Returning Home easier than ever thanks to Three Rivers

Home Sweet Home is a simple trio of words strung together. But if you or a loved one has had to call a medical care or nursing home facility as your temporary residence, getting back to that “Home Sweet Home” is so dearly important. 

Three Rivers Regional Commission and its Area Agency on Aging can help get you back home.

Here in Coweta County, the local agency office is located in nearby Franklin. The Three Rivers Agency on Aging offers support services that many folks do not know about. Three Rivers can assist you or your loved one in transitioning from that temporary stay at the medical care facility or nursing home with a pair of programs.

Those programs – Money Follows the Person (MFP) and Nursing Home Transition (NHT) – can assist with the move from those facilities and put support services in place which are uniquely designed to meet an individual’s needs. Best of all, it keeps you or that loved one living safely in your home. Those services can range from monetary assistance to removing difficult challenges like transportation issues that may arise and more.

What choice is right for you?

With the Money Follows the Person program, individuals must be in a facility for at least 60 consecutive days; must receive one day of Nursing Home Medicaid; and must enroll in a Medicaid Waiver.

The Nursing Home Transition programs requires individuals to be in a facility for at least 20 consecutive days; must be 55-or-older; must be a U.S. citizen or have legal status; and monthly income must be below $6,175.
